The Opening Ceremony for the new Patient-Centered Medical Homes in Fort Campbell will be held at 2 p.m. on Friday, June 7. The event will also serve as the Open House for the facility, which previously contained the Blanchfield Army Community Hospital’s family primary care services.

This event formally illustrates the efforts of The Army Medical Department to expand the medical home model to all primary care settings throughout the Army as of November 2011, when the first Community-Based Medical Home was opened by the Army.

The transformation of the medical building is intended to be a positive one, with the model followed by the transformed facility being referred to by those from Blanchfield Army Community Hospital as one that “offers patient-centered, team-based, holistic care, which involves the patient as the center of the care model” that “has shown to improve patient satisfaction.” With PCMH, patients will ideally be seen only by their individual care teams rather than any provider with availability, and will benefit from the use of telephone consultations and secure email messaging.

This PCMH, referred to as the Screaming Eagle Medical Home, is located next to Gateway Medical Center in Medical Office Building One, will continue to serve the more than 30,000 patients formerly assigned to the Blue, Young Eagle and Byrd Family clinics. The new clinics will be the Air Assault Family Medical Home, the Young Eagle Medical Home, and the Byrd Family Medical Home.

This change follows the transformation of BACH’s internal medicine clinic, the Gold Clinic, to the Gold Army Medical Home, adopting the PCMH model in August 2012.
